Recommended Consent Letter for Children Travelling Abroad

The Travel Consent Letter is required for all players under the age of 18 traveling without their parents or legal guardians. If this form is not submitted and is required, please be aware that your child may encounter complications at the airport, and you assume full responsibility for any related issues.

If your child is traveling with an accompanying parent/legal guardian, or is over the age of 18, check the box below to skip the Consent Form upload requirement.
